DOT Physicals

As of May 21, 2014 DOT physical examinations can only be conducted by a licensed medical examiner.

DOT physicals are physical examinations and drug screens required by the Department of Transportation for drivers carrying commercial loads on public roads. In order to pass the examination:

  • Vision must be 20/40 with or without correction (glasses/contacts)
  • Blood pressure must be less than or equal to 140/90
  • Hearing must be strong enough to hear a loud whisper from several feet away

Insurance is not accepted for DOT physicals and payment is due at the time of your visit.

Prices are as follows:

  • Exam: $120
  • Urine Test: $20

How to prepare for your physical:

  1. Bring eyeglasses/contact lenses/hearing aids if you use them.
  2. Have a list of your current medications as well as the name and contact number of the doctor who prescribed them.
  3. Complete the Driver Information and Health History sections of the Medical Examination Report form. Do not forget to sign and date the form.
  4. Supply us with medical records for conditions such as diabetes, cardiac disease, sleep disorders, neurological conditions, etc. If you take medications for pain, psychiatric conditions, attention deficit disorder, or smoking cessation for example, bring a letter from your treating doctor.
